The Schwerbehindertenausweis (severe disability card) shows that you have a severe disability. You get it from a Grad der Behinderung of 50.

You apply for it at the Versorgungsamt (pensions and disability office), which has another name in some places. Enclose reports from your doctors.
The card gives you certain rights, for example more holiday at work or special protection against dismissal.
